Severe Adult Needs
The ACT Team
The ACT model is indicated for adults with severe and persistent mental illnesses that produce distress and major functional disability in adult role functioning (self-care, social and interpersonal relationships, employment, and navigating the community). This model includes a combination of intensive therapy, medication management, and case management services utilizing a multidisciplinary team approach. These individuals may have had multiple hospital admissions or frequent involvement with law enforcement.
In order to be considered for the ACT team, individuals should meet the diagnostic criteria for one of the following:
- Schizoaffective Disorder
- Bipolar Disorder with severe with psychotic features
- Major Depression with psychotic features
While individuals may qualify diagnostically they also MUST have failed at traditional outpatient therapy and case management services.Due to the intensive nature of the ACT model, individuals must live in Tippecanoe County in order to receive services from the ACT Team.
